Job summary

The Palliative care clinical nurse specialist will In-reach with the acute hospital areas of Calderdale & Huddersfield NHS Trust (A&E, Acute floor and SDEC), alongside an allocated palliative care social worker to identify and facilitate the rapid discharge or transfer of patients with palliative care needs to an appropriate preferred place of care. The post holder will support timely, effective care, assessment, and intervention for patients with specialist palliative care needs with in a multi-professional workforce.

Main duties of the job

The Palliative care clinical nurse specialist will In-reach with the acute hospital areas of Calderdale& Huddersfield NHS Trust (A&E, Acute floor and SDEC), alongside an allocated palliative care social worker to identify and facilitate the rapid discharge or transfer of patients with palliative care needs to an appropriate preferred place of care. The post holder will support timely, effective care, assessment, and intervention for patients with specialist palliative care needs with in a multi-professional workforce.

Job responsibilities

Clinical Duties

The successful candidate will:-

Work within the multi-professional team to manage the caseload of patients presenting with specialist palliative care needs in the acute setting (Emergency Department, Acute floor and SDEC)

Be able to triage and holistically assess patients with life-limiting conditions, that may not necessarily require prolonged stay in hospital and facilitate a rapid discharge to preferred place of care.

Provide symptom management advice.

Be able to triage, assess and escalate patients with specialist palliative care needs to the Hospital specialist palliative care team when necessary.

Work alongside an allocated palliative care social worker to facilitate social care and FT funded needs.

Liaise and work alongside the multi-professional teams including liaison with community palliative care team to support patients' needs to ensure admission avoidance in acute setting and from home.

Act as a resource for sign posting, patients, families, carers and health professionals about palliative care services in Hospital and in the community.

Provide guidance and support for patients making choices relating to preferred place of care and advance care planning.

Involve patients, their families and carers in the planning and delivery of their care.

Assess and facilitate rapid assessment to hospice when appropriate.

Support advance care planning as appropriate and record in System One EPaCCS when consenting to share information.

Will support integration of end-of-life care tools (eg. SPICT / Gold Standards Framework)

Have the ability to audit performance of the project in order to participate in service review and development alongside you Macmillan palliative care team leader.

Professional Duties

Work within the NMC Code of Conduct at all times.

Ensure adherence to all CHFT protocols, policies and directives within service delivery.

Act as a role model for quality nursing care at all times, promoting the need for effective evidence-based interventions and compassionate care.

Demonstrate specialist skills and knowledge.

Act as patient advocate at all points of the care pathway.

Managing self

Participate in regular supervision.

Attend all mandatory training.

Participate annually identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal.

Comply with all Trust policies, procedures and protocols.

Carry out duties with due regard to the Trusts Equal Opportunity Policy.

Seek advice and support from Line Manager whenever necessary.

Maintain professional conduct including appearance at all times.

Ensure maintenance of Professional Registration.

Managerial Duties

Participate in service review and development alongside the specialist palliative care team.

To ensure that continuity of care for patients and appropriate specialist nursing or other health and social support is available and accessible as appropriate, liaising where appropriate with other care settings and professionals.

Work with other disease specific clinical teams to ensure equity of access to specialist palliative care for patients with both malignant and non-malignant conditions.

Support CHFTs aims and objectives around service delivery and achieving targets.

Working with the Specialist palliative care nursing team by taking part in weekly Clinical review meetings to discuss complex cases, reflect and ensure best possible outcomes.

Promoting excellence in palliative care via evidence based practice with measurable standards of care and outcomes. Have involvement in audit projects to measure standards of care.

Help to identify and highlight any gaps in the evidence/service provision under the supervision of the Macmillan specialist palliative care nursing team leader.

Attend regular relevant meetings (internal and external) and provide effective channels of communication within the team as required.

Maintain a flexible approach and develop the role according to needs.

Work alongside the Macmillan specialist palliative care nursing team leader to support the division/Trusts annual plan and strategic vision in all care settings.

Work towards supporting the Trusts cost efficiency programme by correct use of resources and identifying areas for further improvement.
