Job summary

The Named Nurse role is integral to delivering the statutory health service to Children Looked After (CLA) and Care Leaver's (CL's) in the care of Calderdale Metropolitan Borough Council (CMBC). Incorporated in the duties undertaken is the promoting and enabling of CLA and CL's to achieve their optimum outcomes for health and wellbeing.

The Named Nurse will operationally manage the delivery of health services for Children Looked After (CLA) and care leavers (CL's) for Calderdale as outlined in Promoting the Health and Wellbeing of Looked after Children (2015) and demonstrate Level 4 competencies outlined in Intercollegiate Competencies for Looked After Children (2015)

The Named Nurse for CLA is a high level operational role and plays an important part in promoting good practice within the provider organisation.

The Named Nurse for CLA provides a high level of operational leadership and expertise and holds line management responsibilities for the Children Looked after team which aims to improve the health outcomes for CLA, and will ensure that there are systems in place to meet the health needs of CLA and CL's in a robust and timely manner.

The role will provide a safe and effective operational service to support CLA and young people to reach their full potential.

Main duties of the job

Engage in effective strategic planning of services for looked after children

Identify and take responsibility for developing, implementing and reviewing policies, procedures and quality standards that reflect statutory requirements and recommendations of national guidance for looked after children

Monitor trends, quality and appropriateness of referrals, analysing and identifying gaps, duplications, and barriers to systems and take appropriate action.

Be able to work effectively on an inter-professional and interagency basis developing active partnerships with other organisation boundaries in the interest of CLA, CLs and their carers

Be able to identify unmet health needs/gaps in service provision and promote innovative service solutions that demonstrate CLA & CLs informed choices wherever possible

Know how to ensure legal processes and requirements for looked after children including after care are appropriately taken.

Be able to advise other agencies about the health management of Looked after children and Care Leavers.

Be able to apply lessons learnt from audit, case management reviews, significant case reviews to improve practice.

Be able to participate in and chair multi-disciplinary meetings as required.

Work closely with the Designated Nurse and Doctor for Looked after children contributing to the strategic development of services for CLA.

To communicate highly confidential, highly complex and in some circumstances highly contentious information to multidisciplinary colleagues and carers.

Collect, collate and disseminate health data for CLA as appropriate, producing times function specific reports as required using prolonged concentration and appropriate software packages when required.

Demonstrates an understanding of appropriate and effective training strategies to meet the competency development needs of different staff groups

Promote effective consultation with other healthcare and social care professionals including participation in multi-disciplinary discussions/meetings.

Demonstrates an understanding and experience of developing evidence-based clinical guidance

Identify, design and deliver training and supervision for multiagency professionals and carers.

Demonstrate critical insight of personal limitations and an ability to participate in peer review

Take responsibility of staff that assess, plan and contribute to the implementation of health action plans for vulnerable children, ensuring that all staff are engaged

Provide day to day line management of the CLA team, providing leadership and objective setting and that appraisals are in place for each staff member, whilst dealing effectively with staff related issues

Monitor and highlight area of risk for discussion at meetings and bring to the attention of senior managers and commissioners.

Manage the development, management and monitoring of contracts or service level agreements, reporting against set KPIs and targets.

Working closely with social care managers and teams to ensure effective communication.

Participates in audit, and in the design and evaluation of service provision, including analysis of findings, the development of action plans and strategies to address any issues raised by audit and serious case reviews/internal management reviews/significant case reviews/other locally determined reviews related to looked after children

Manage the service within a financial budget ensuring that there is efficient use made of available resources.

Identify and act upon any trends/risks or shortfalls relating to these budgets, seeking the involvement of the Service Manager where appropriate.
