Job summary

We're excited to be recruiting to pilot a new role that will help shape the future of stroke care. This position will pioneer innovative ways of working to ensure our stroke patients experience truly seamless support -- from their time in hospital right through to recovery at home. Main duties of the job

The teams are multidisciplinary in nature and so the role will work alongside Physiotherapists, Speech and Language therapists, Dietitians, Specialist stroke nurses, and rehabilitation support staff.

The post will follow the patient's journey from acute therapy areas, through discharge from hospital, into a suitable community setting supported by the ESD service, therefore, due to the nature of the role the successful applicant would hold a full UK driving licence and have access to a vehicle. However, if this is not possible due to a disability defined by the Equality Act 2010 then reasonable adjustments would be considered.

The successful candidate will have experience in treating stroke patients and have specialist assessment and rehabilitation skills that are transferable across acute and community settings.

The postholder:-

To work within the Stroke ESD pathway to ensure seamless rehabilitation on discharge home from hospital, and on transfer to long-term community services.

To develop specialist Occupational Therapy interventions in line with current evidence base.

To manage autonomously a clinical workload of patients / clients at a specialist level, assessing and treating patients, and maintaining clinical records.

To have specialist assessment and treatment skills in the areas listed but not limited to:

Stroke

Cognitive, physical and perceptual deficits

Anxiety management

Complex psychosocial assessment of patients and carers

Assessment for assistive equipment and adaptations

Assessment of the home environment

Splinting and orthotics

To apply evidence based knowledge within the specialist clinical area acquired through experience and academic study to a specialist post graduate level.

To act as a resource and provide advice for, other professionals and colleagues within the MDT about the assessment, treatment, management and care of patients / clients who have suffered a stroke.

To work collaboratively with health and social care colleagues in community and acute settings.

To work with other members of the MDT to ensure effective care pathways and transfer of care / discharges.

To develop specialist programmes of assessment, treatment and intervention for patients within a community and inpatient setting.

To manage and prioritise own clinical workload as appropriate, working efficiently to manage professional time and delegate according to the training and competency levels of other team members to ensure the effective delivery of the service.

To participate in the supervision process with staff on a daily basis and for students as required.

To To ensure a high standard of patient care is provided including implementing and reviewing clinical standards relevant to the work undertaken.

To provide specialist assessment and advice for the multidisciplinary team and actively participate in MDT meetings as appropriate.

To work across a 5-day therapy service.

  • Relevant Degree/Diploma
  • Registered with HCPC
  • Willingness to undertake additional and mandatory training as required.
  • Willing and able to participate in a competency work based training programme

Desirable

  • Evidence of postgraduate training
