Clinical Support Worker - Special School Nursing Team

Job summary

An exciting opportunity has arisen for a highly skilled and motivated individual to work in the Specialist School Nursing Team.

This is a term time role and the successful candidate will be required to work across all 5 sites if needed, on a 4 day/week roster. Hours of work are 0830-1600

The SSNT work in 5 specialist provision schools across Calderdale and Kirklees managing and supporting a caseload of children with medical and nursing.

We are looking to recruit someone into this position who has experience at caring and supporting children and families with complex medical and nursing conditions.

The individual will work as part of the Specialist School Nursing Team as a clinical support worker in providing compassionate care to pupils, service users, carers and families. This is a pivotal role that is essential in providing care and support to pupils during the school day to support their medical and nursing needs in school. Through direct and indirect care, working within clearly defined limits and attainment of agreed competence.

Be able to work without constant direct supervision using initiative within the level of competence for the post.

Main duties of the job

We are looking for candidates who have previous experience working with children with complex medical and nursing needs.

The children we care for can have long term medical conditions and will require nursing support during the school day to maintain their health and wellbeing. Experience working with children and young people with learning disability required.

Job responsibilities

Must be able to undertake other tasks appropriate to the clinical environment with necessary training and as directed by the Ward Manager or Department Lead:

Supporting the delivery of nursing care within the school day.

Supporting the qualified staff nurse on site with management of emergencies as well as administration of medication following the Trust policy to support this.

Completion of the competency document relative to the role.

Recording of patient information on relevant systems for the role.

Undertake physiological observations: blood pressure, pulse, temperature, respirations and oxygen saturations as directed by the nurse in charge and record.

Support the Registered Nurse in the delivery of nursing care in school

Attend training in the use of equipment and devices appropriate to role and reports faulty equipment promptly.

Record all care provided in the Trust standard format.

Keep equipment cleaned and labelled.

Participate in Intentional rounding ensuring that skin care is undertaken and comfort is maintained, recognising, reporting and recording changes to condition of skin.

Order equipment as directed by the registered nurse using appropriate system.

Empty drainage bags such as catheter bags, stoma bags using appropriate infection prevention and control precautions.

Test patient blood glucose levels from capillary samples recording results and escalating when required.

To collect specimens of urine, sputum or faeces as requested and according to ward/ department procedures.

Remove cannulas and apply dressings as directed by the nurse in charge.

Participate in audit activities at the request of the Ward manager / Department lead

Act as a chaperone to support privacy and dignity of patients during intimate cares and examinations.

Provide one to one care for patients under the direction of the Registered Nurse participating in appropriate engagement activities to support plan of care.

Complete MRSA screening of patients as instructed by the Registered Nurse.

The post holder will be provided with the relevant training and knowledge to use and apply a range of wound dressings and closure techniques, under the direction of a registered nurse.

The post holder will be responsible for documentation within the electronic patient record, under the direct / indirect supervision of a registered nurse.

To be competent in and undertake other tasks appropriate to patient care that is delivered within the care setting. These tasks would be under the direction and supervision of the nurse, examples but not limited to:

o Undertaking an ECG.

o Application of a plaster cast.

o Venepuncture.

o Cannulation.

o To perform clinical tests i.e. Point of Care Testing

o Emptying and changing drainage bags i.e. stoma bags

o Catheter care removal of catheters, obtaining urine specimens.

o Undertakes delegated care without direct supervision where appropriate this may require the use of observation / analytical skills to monitor the health and wellbeing of patients.

o Measuring and supplying TED stockings.

o Testing visual acuity.

o Undertake pregnancy tests.

Understand importance of individual role and contribution in ensuring that pupil, service users and carers have a positive experience of care.

To ensure appropriate escalation of concerns and use of incident reporting system.

Take responsibility for adapting communication style to meet the needs of pupils, service users and carers adopting basic de-escalation techniques when needed.

Ensure up to date knowledge of risks in the area and any key safety messages as communicated by the Ward Manager/ Department Lead.

Ensure active participation in safety initiatives in the ward or department such as safety briefs.

Ensure compliance with infection prevention and control practices reporting any concerns to the nurse in charge.

Understand own contribution in creating a psychologically safe environment by displaying appropriate behaviours and treating colleagues and service users with respect and civility.

Ensure that call bells are responded to in a timely way and that intentional rounding is completed in line with local guidelines.
