Job summary
This is an exciting opportunity to work within the CHFT multi-disciplinary specialist Cardiac Rehabilitation / Heart Failure and pulmonary rehabilitation team. The post holder will deliver community based Cardiac and Pulmonary rehabilitation service that supports and promotes self-management. The rehabilitation is delivered to groups in community settings, one to one in patients own homes or regular reviews on the telephone and as part of a supported discharge pathway for respiratory patients. There will also be opportunity to provide virtual rehabilitation using digital and remote platforms.
Main duties of the job
The post holder will act as a specialist Physiotherapist for the Cardiac Rehabilitation / Heart Failure team and the Pulmonary rehabilitation team. They will contribute to embedding evidence-based practice into the clinical area. The post holder will work flexibly to support the needs of both services. The post holder will use shared decision making and offer a range of treatment modalities to promote independence and improve quality of life for patients.
