We are delighted to offer a new and exciting opportunity for a dynamic, experienced nurse to join our friendly and well-established multidisciplinary Invasive Cardiology team as a Cardiac Rhythm Management (CRM) Specialist Nurse. This newly created post reflects our commitment to expanding and modernising cardiac services within the Trust.
We're looking for a motivated nurse with a cardiology background, excellent clinical skills, and the ability to work collaboratively across disciplines. You'll play a key role in delivering a high-quality, patient-focused CRM service, working closely with the Invasive Cardiac Physiologists and wider teams both locally and regionally.

Main duties of the job
- Will primarily undertake CRM combined nurse and physiologist led clinics providing a full clinical assessment, physical examination, and adjustment of medical therapy for this cohort of heart failure patients.
- Assess, diagnose, and manage patients with cardiac rhythm disturbances with the support of the wider multidisciplinary team. This includes advice regarding medications, making referrals to other members of the multidisciplinary team, requesting investigations.
- Monitor and interpret data from implantable cardiac devices using specialist platforms (e.g., Merlin, CareLink, Latitude).
- Support pre-assessments, post-procedure, and follow-up care for CRM device implant patients, including medication management. Prescribe (or instruct primary care) and safely adjust cardiac medications, including heart failure, anti-arrhythmic and anticoagulants drugs, within the scope of cardiology independent prescribing.
- To be BHRS accredited and be willing to study towards the BHRS Nurse accreditation.
- Participate in CRM multidisciplinary team (MDT) meetings and contribute to complex case discussions. Triage patients to appropriate MDTs. Action agreed plans and document on EPR.

The postholder: -
- Will primarily undertake CRM combined nurse and physiologist led clinics providing a full clinical assessment, physical examination, and adjustment of medical therapy for this cohort of heart failure patients.
- Demonstrate a high level of expertise within the CRM specialist service providing advice, education and support to staff, patients, their families and carers.
- Have responsibility for the on-going development of clinical practice and standards of care within the CRM service, including the development of evidence-based policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues.
- Be practising autonomously as a nurse practitioner within the designated speciality area to provide patient-centred clinical care. This will encompass the skills of clinical assessment, examination, diagnosis and treatment within an agreed scope.
- Support new ways of working that emphasises a more efficient and patient-focused service, and will ensure the safe treatment, referral and discharge of patients within CRM.
- To liaise and engage with wider teams within community, primary, secondary and tertiary care to establish care pathways that support the referral and management of patients within CRM in respect to service development, referral and access to services.
- Collaborate with the wider cardiology team to ensure patients are assessed and a plan of care is in place which is relevant to the patients ongoing safe and timely care, including the prompt prescribing of medication and assisting with discharge. Focusing on proactive patient care and management.
- To maintain open and effective communication within the invasive team and senior cardiology doctors with the patient journey in mind.
- Assess, diagnose, and manage patients with cardiac rhythm disturbances with the support of the wider multidisciplinary team. This includes advice regarding medications, making referrals to other members of the multidisciplinary team, requesting investigations, managing emergency situations and arranging emergency clinic visits. To provide education, counselling and reassurance to patients and their families within your care.
- To assess patients psychological needs and refer to wider support teams as necessary. To provide expert clinical advice and support to nurses and wider members of the multidisciplinary team and act as a clinical role model.
- To assess patients clinical status and be willing and able to discuss end-of life care and deactivation of defibrillators when at advanced stage of heart failure/deteriorating health.
- To be a point of contact for clinical teams within the Trust for patients admitted with a CRM implantable device, providing prompt independent assessment and advice in collaboration with the Cardiac Physiologists ensuring high standards of professional patient centred care.
- Monitor and interpret data from implantable cardiac devices using specialist platforms (e.g., Merlin, CareLink, Latitude). Respond to remote monitoring alerts, Triage HF and determine appropriate clinical actions. This training will be given and supported by the Cardiac Physiologists and wider CRM training opportunities.

- Attend local and regional meetings, CRM training events and conferences within area of expertise and maintain CPD.
