Job summary

The post holder will work as in integral member of the Neurology Service, providing an exemplar service for patients diagnosed with multiple sclerosis who have been prescribed disease modifying therapy.

Main duties of the job

The MS DMT coordinator role supports patients who are diagnosed with multiple sclerosis and have been prescribed disease modifying therapy (DMT), the MS nurse specialist and the MS specialist consultant neurologist. Through proactive management, being the single point of contact for MS patients, the role contributes to improving patient experience. Key to this role is extremely strict organisation of nurse monitoring appointments, consultant review appointments and corresponding blood tests. It is imperative that DMT patients are closely monitored and their care co-ordinated.

Job description

Job responsibilities

The postholder will:

The post-holder will co-ordinate activity within the Neurology Team for MS patients, specifically Disease Modifying Treatment patients.

The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.

Utilise advanced level IT skills to create reports, analyse data, compile spreadsheets and presentations.

Responsible for communicating effectively with service users and their families.

To deal in a professional, helpful and sensitive manner with service users, staff and other agencies by telephone or face to face, taking messages, resolving concerns, advising service users about visits, referring other issues as appropriate and answering routine enquiries.

To keep an up to date and strict database of DMT MS patients, ensuring that actions have been taken in relation to blood requests, blood results, monitoring and ensuring that follow up appointments are booked, all to ensure patient safety.

Delivering consistent access to the service, triaging and escalating patient and staff enquires to the department and liaising with the nurse specialists as appropriate.

To extract and collate information for audit and service development/improvement specifically in relation to MS patients who have been prescribed DMT medication.

Responsible for maintaining effective communication channels across both sites, linking with key departments, MS consultants, Leeds Trust MS Service, and healthcare professionals.

Accurately monitoring each DMT MS patient journey to ensure appropriate care.

Supporting the delivery of information to ensure that all MS patients and their carers receive appropriate verbal/written communications in a timely manner.

Document and monitor all aspects of care coordination and service delivery

Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to care.

To organise and book patients into appropriate MS clinic slots on the EPR.

Project manage patient education events and educational sessions internally and externally including regional forums for MS specialist nurses.

Actively participate in the provision of routine information and advice and refer patients to appropriate agencies in both primary and secondary care as directed by the MS specialist nurses and MS consultants.

To prioritise workload on a daily basis and deal with the competing demands of the integrated community teams..

Be responsible for updating and maintaining databases to track service users case management.

Responsible for advising line managers of nurse specialist sick leave and arrange transfer of patients accordingly.

Record and monitor, electronically and manually, all neurology nurses leave, such as annual leave, professional leave, study leave etc.
