Job summary

This is an exciting time for Calderdale's district nursing teams as we have established 5 community hubs each with its own unique and diverse needs.

We are seeking enthusiastic staff members with excellent communication skills, who are highly motivated and adaptable in a fast paced and ever changing environment.

Due to the nature of the role it is essential that you hold a full driving license with access to a vehicle suitable for business use.

We have a vacancy at Beechwood Health Centre for a full time position with the Unplanned Care Team. This service is essential in ensuring that patients recieve timely and effective care, preventing unecessary hospital admissions and supporting patients in their own home.

- To participate in the assessment, development, implementation and evaluation of programmes of care. Calderdale and Huddersfield Foundation Trust provide a 24 hour district nursing service and, as such, an element of flexible working is expected to maintain safe, effective, responsive services.
