Job summary

Calderdale and Huddersfield NHS Foundation Trust are seeking an enthusiastic advanced clinical practitioner to join our Urgent Community Response (UCR) and Same day Emergency Care team delivering care to patients closer to home and preventing unnecessary hospital admission across Calderdale community. . The advanced clinical practitioner role is an innovative and dynamic way to work with people to support them to receive high quality and advanced level care in their own home. This role is part of an exciting development to deliver an integrated admission avoidance services in conjunction with Primary care, social care, Yorkshire Ambulance Service South Central Ambulance and the acute trust.

The Advanced Clinical Practitioner will be a highly experienced, knowledgeable and educated member of the team who is able to work autonomously making high level decisions to treat health needs.

Main duties of the job

  • Assess individuals, families and carers holistically, using a range of different methods, some of which may not be usually exercised by nurses such as physical assessment and history taking, ordering and interpreting diagnostic tests or advanced health needs assessments
  • Communicate effectively with patients' carers and the multidisciplinary team.
  • Use their expert knowledge and clinical judgement to decide whether to refer patients for investigations and make diagnoses.
  • Decide on and carry out treatment, which may include the prescribing of medicines, or referring patients to an appropriate specialist.
  • Ensure the provision of continuity of care.
  • Assess and evaluate, with patients and /or carers, the effectiveness of the treatment and care provided and make changes as needed.
  • Work autonomously within a wider health care team and as a lone practitioner.

Job responsibilities

  • To work closely with Primary Care, LCD, community and acute medical teams to provide effective and reactive services for people presenting unwell in their own home to ensure rapid delivery of treatment, care planning and hospital admission prevention where appropriate.
  • Practice as an autonomous professional in a self-directed manner, with confidence, authority and expertise, undertaking comprehensive range of clinical procedures.
  • Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made.
  • Work clinically to optimise health and wellbeing, through comprehensive history taking and physical assessment of patients.
  • Drawing on a diverse range of knowledge and skill, determine appropriate therapeutic interventions,
  • Plan, prioritise and manage complete episodes of care, for patients presenting with undifferentiated and undiagnosed conditions.
  • Be proactive in clinical decision making, underpinned by an advanced level of theoretical and practical knowledge and be able to demonstrate improved patient care outcomes.

  • To advise, communicate and work in conjunction with a wide range of clinical colleagues specifically, primary care and community teams and Social Care professionals, leading and facilitating a patient or client focused, coordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.
  • Take a lead in empowering and facilitating other professions to meet the care needs of service users and their carers by the promotion of principles and philosophies such as care closer to home, rehabilitation and optimal self-care.
  • To proactively participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patients own home as possible.
