Job summary

A rare and exciting opportunity has arisen for 1 dynamic and driven Occupational Therapists to join the Acute Therapies Teams in a clinical leadership role at Huddersfield Royal Infirmary and Calderdale Royal Infirmary. This role has the potential to have some elements of community based work. The team comprises of a number of clinical specialities including A&E, medicine, respiratory and surgery, and associated 'front end' areas. Candidates will therefore bring a breadth of experience to the role - we are particularly keen to see applicants with a background in acute hospital care including medicine, elderly, orthopaedics and 'front end`/ A&E experience. The team consists of Physiotherapists, Occupational Therapists and support staff and will be jointly led by the successful applicant and the established clinical lead physiotherapist. The successful applicant will be joining the service at a time where we are reviewing and developing our service delivery, so this is an excellent opportunity to be part of service development and change management, which will be supported by a strong supervision structure and a dedicated senior management team. We provide a service across a 7 day rota which the successful candidate will be expected to participate in

Main duties of the job

To act as the Clinical Lead for the Occupational Therapy Service with Medicine and Elderly patients, advancing clinical practice in this area of work and taking a lead in evolving and embedding best practice into clinical To manage own highly specialist caseload in this area. Work autonomously within Trust policy and towards Hospital Discharge and Community Support Guidence (DoH). Maintain professional practice and accurate and comprehensive up to date records. To perform highly specialist assessment and treatment of acutely ill patients in A+E , Medical Assessment unit, Short stay unit and medical and elderly care wards, with the aim of preventing hospital admissions and/or facilitating safe and timely discharges and transfer of care. There will be at times a requirement to take part in home visits and access visits. Determine a clinical diagnosis, develop, deliver, implement, evaluate and modify individualised treatment programmes and/or highly specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required. Educate and train Occupational therapists and other qualified and non-qualified health and social care staff including medical staff. To assist the Team Leader in the daily operational management of the Medicine and Elderly Care team. To support and advise within the wider MDT on the delivery of the service

CHFT employs over 6,000 colleagues who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital (CRH) and Huddersfield Royal Infirmary (HRI) as well as in community sites, health centres and in patients' homes.

In December 2018 the Department of Health and Social Care awarded CHFT £196.5m to invest in local health services. This investment will be used to build new buildings on both sites including a brand new A&E and Paediatric A&E building, a new Enhanced ICU and an upgrade to theatres wards. A new staff and patient car park will also be built.

To manage autonomously a clinical workload of Medicine and Elderly care patients at a highly specialist level, assessing and treating patients and maintaining clinical records. To have highly specialist assessment and treatment skills in the areas of, but not limited to: Cognitive and perceptual deficits, physical rehabilitation through exercise and function, crisis intervention, falls assessment, home visits, access visits, early supported discharge / discharge facilitation, positioning and seating. Assessment for assistive equipment and adaptations, anxiety and fatigue management including cognitive behavioural therapy, assessment of the home / discharge environment. Discharge and transfer of care planning and co-ordination To apply evidence based knowledge within the specialist clinical area acquired through in depth experience and academic study to a specialist postgraduate level. To act within the acute therapy service as a resource, and provide advice for Occupational Therapists and other professionals within the MDT about the assessment, treatment, management and care of patients who have a multipathological illness and/or complex psychosocial needs. To work collaboratively with health and social care community and acute colleagues. To take a lead, and work with other members of the MDT to ensure effective care pathways and transfer of care/discharges. To proactively and appropriately facilitate a seamless transfer of care and continuing therapeutic management of Medical and Elderly patients between the hospital and community settings. To work collaboratively in an interdisciplinary manner within the MDT, being able to debate the blurring of boundaries across professions and where competent take on aspects of blurred boundary working. To develop specialist programmes of assessment, treatment and rehabilitation and care for Medical and Elderly patients in the acute hospital setting and within a home environment / discharge setting. To manage and prioritise own clinical workload as appropriate working efficiently to manage professional time and delegate according to the training and competency levels of other team members to ensure the effective delivery of the service. To ensure a high standard of patient care is provided including implementing and reviewing clinical standards relevant to the work undertaken. To provide highly specialist assessment and advice for the multi-disciplinary team and actively participate in M.D.T. meetings as appropriate.
