Job summary

An exciting opportunity has arisen for a Community Staff Nurse to join our West Berkshire Community Nursing Team.

Our service is evolving rapidly, caring for patients with increasingly complex and acute needs, providing specialist treatments and high-quality, evidence-based care in the comfort of their own homes.

As part of our Community Nursing Team, you will assess, support, and treat patients with a wide range of health needs. You will demonstrate compassion, professionalism, and a strong commitment to delivering excellent patient care. This role is ideal for someone with a genuine passion for community nursing, helping patients maintain their health and independence at home.

The service operates 7 days a week, 08:30-19:00, inc. Bank Holidays, providing care across the West Berkshire area. Our team includes a diverse skill mix, from Band 3 Healthcare Assistants to Band 7 Community Matrons, ensuring a supportive working environment.

To succeed in this role, you will need to demonstrate a commitment to holistic, patient-centred care, the ability to work independently and as part of a multidisciplinary team, and strong clinical skills including complex wound care, bladder and bowel management, palliative and end-of-life care, and medication administration.

You will be fully supported through induction, ongoing training, and professional development opportunities, with clear pathways for career progression.

Main duties of the job

  • To work in conjunction with the Team Leader/Community Nursing Band 6/Community Matron and all staff within the team to holistically assess the needs of patients and carers, and to work in partnership with the patient and family to plan, implement and evaluate programmes of nursing care.
  • To be able to undertake first visits and take any immediate action, reporting back to the Team Leader/Community Nursing Band 6/Community Matron for further discussion and assessment.
  • Toestablishworking relationships with other professionals and voluntary agencies to achieve health aims.
  • To undertake practice assessor and supervisor training support students andnew staffto the service.
  • Provide care according to the available evidence of best practice and the support of others in the development ofevidence-basedpractice.
  • To be actively involved in promoting self-care for patients and building Health Promotion into care.
  • To develop andparticipatein teaching programmes for patients,
  • Todemonstrateknowledge and skills including assessment, reassessment, medication management, bladder bowel care, tissue viability, care,useand administration of treatment using specialist equipment, palliativecareand symptom control, managing acute episodes of care and reducing hospital admissions.
  • To provide care to people with long term conditions undertaking reviews in accordance with the individual patient care plan and with support of more experienced colleagues as required.
