We have a fantastic opportunity to recruit an experienced Nurse for an Advance Nurse Practitioner role or Advanced Clinical Practitioner to join the West Berkshire Urgent Community Response / Virtual Ward Team.

The role will provide advanced clinical assessment to patients in their own homes and in a care homes, who require an urgent response to prevent unnecessary hospital admission. The post holder will work as part of a multi-disciplinary team to provide appropriate patient focussed care.

The role will provide leadership and line management to junior staff within the multidisciplinary team, including nurses, paramedics, and support workers, and clinical supervision and training/ education to the nurses, paramedics, and support workers, as required.

You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.

The post holder will work 37.5hrs per week, 7 days a week, between the hours of 8am - 8pm, which will include weekends.

The role is based at Hillcroft House, West Berkshire Hospital

Main duties of the job

  • To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service in collaboration with theCommunityGeriatricianand GP.

  • As an experienced advanced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis,treatmentand management of complex patients considering local and national guidelines.

  • The postholder is expected to liaise effectively and professionally with all levels of staff, externalorganisationsand the public, dealing with confidential information inan appropriate manner.

  • As an Independent Prescriber, prescribeand reviewmedicationsin accordance withNMCguidance, local and national policy guidance.

  • To provide leadership and line managementto the Band7nursesandtherapists andthe Band3 &4 healthcare/multi-therapy assistants.

  • Ensureaccurate, contemporaneous documentation and care plans aremaintainedaccordance with theNMC, information governance and local / national guidance.

  • Work collaboratively with colleagues and develop nursing and care delivery policies,identifyinghow policy willimpacton practice and service delivery.

  • Work flexibly in response to service need across our multiple sites
