You will work within theBracknellarea to deliver core and targeted services to children aged 0-5 and their families.

For this role you will requireexcellent communication and interpersonal skills as well asa passion for working with children and families.

Team working, flexibility andthe ability to work in partnership with other teams and agenciesis essential. You will collaborate with Children'sCentre'sin the implementation of the Healthy Child programmeunderpinned by the Solihull Approach model of working.

Ifsuccessful,your base will beWokingham Community Hospital;however,your role will require you to visithomesacross Berkshire (mileage will be compensated).

Main duties of the job

Some of the general responsibilitiesinclude.

  • Develop and implement outcome focused care plans and evaluate care given, in partnership with clientsin accordance withthe Healthy Child Programme.

  • Provide support to families using the Solihull approach model in behaviour management, parentingskillsand delivering programmes of care to vulnerable families.

  • New birth visits, targeted and safeguarding workinin collaboration with our partners,supporting parents with infant feeding,attendance at strategy and multi-professional meetings.

  • Develop andfacilitateHealth Visitingdrop-inclinics, health promotionactivities and groups.
