Main duties of the job

  • You will visita number ofpatientson a daily basisin their ownhomes.

  • You will provide high quality nursing care to a range of patients with varying healthcareneeds.

  • Deliver clinical and therapeutic care to patients within agreed trust policies/ protocols, as documented in the band 3 competency assessment tool.

  • Contribute to the holistic assessment and management of patients care needs throughinitialdata collection, monitoring ofpatientsprogress, feedback discussion reports and in care reviews with the Community Nursing Team.
