Are you an experienced Children's nurselooking for a new and exciting community-based role, working within a supportive and friendly team?

We are looking for someone new to join our team on a 1 year FTC and are open to both full and part time applicants.

This an exciting and varied role and we are looking for someone with post registration experience in paediatrics, with experience in teaching and assessing.

This is an autonomous role where you will manage your own caseload supporting parents to look after children at home. As a team we strive continuously to improve the lives of those children and families for whom they care, acting as not only clinicians but advocates and advisors too.

You will be based atFir Tree House, Upton Hospital, Slough, covering Bracknell, Windsor, Ascot, Maidenhead and Slough. The service cover for theteamis9am-5pm Mon-Fridayexcluding bank holidays.

Main duties of the job

  • Holding a caseload for children with a wide range of medical and nursing needs acrossBracknell, Windsor, Ascot, Maidenhead and Slough

  • Working within the CCNteam whilstimproving cohesive working between theCCN and specialist school nursing team.

  • Acting as a keyworker for children and families and being the link between hospital and the community. This will include signposting to otherappropriate sourcesof support and liaising with other professionals involved as applicable.

  • Providing excellent standards of clinical nursing care, support and advice to children and their families, including regular reviews, visits and attending clinic appointments as needed.

  • Being part of multidisciplinary meetings with allied professionals, including discharge planning meetings, safeguarding meetings, team meetings etc.

  • Central line maintenance and all associated cares, including taking bloods asrequired.

  • Raising concerns appropriately and being involved with safeguarding processes and attending any Child in Need / Child Protection meetings asrequired.

  • To be part of the Rapid Response rota alongside the Police if a child diesunexpectedly in the community
