Based at Nicholson House, Maidenhead, you'll provide community-based support, visiting clients in their homes. A car is required for travel. While much of our work is virtual, occasional in-person team meetings, training, and service events take place at our office base.

Main duties of the job
Comprehensive Assessments: UndertakingMemory assessments of patients and conduct semi-structured interviews with family members and others involved in a patient's care.Conduct assessments in various settings, including patients' homes, considering physical, psychological, social, and spiritual needs.
Caseload Management: Oversee and coordinate an assigned caseload, ensuring care aligns with the Memory Clinic Care Pathway.
Clinical Support: Assist medical staff in Memory Clinics using assessment tools (MOCA, ACE, etc.) and gathering collaborative patient histories.
Medication & Care Monitoring: Conduct Memory Clinic reviews to track patient responses to prescribed medications and adjust care plans accordingly.
Care Planning & Coordination: Develop, evaluate, and adapt personalised care plans, involving families, carers, and appropriate health, social, and voluntary agencies.
Crisis & Referral Management: Respond to acute psychiatric issues, accept new referrals, and apply therapeutic interventions such as crisis intervention, problem-solving, and Cognitive Stimulation Therapy (CST) techniques.
Liaison & Communication: Maintain strong partnerships with Primary Care, Local Authorities, and Voluntary Organisations to ensure seamless patient care.
Service & Quality Improvement: Contribute to service development projects, ensuring high standards of care and compliance with CPA and Risk Assessment policies.
