We are looking to recruit a Registered Nurse with both Clinical and Management experience, who will have responsibility for the day to day management and clinical leadership of the Urgent Community Response & Virtual Ward team across East Berkshire
The role involves working closely with service users, community-based community services, Primary care, other community health services and social services and the voluntary sector to ensure that the services provided is of a high quality, responsive to local need and demonstrates value for money
You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.
The post holder will work 37.5hrs per week, 7.5 hours a day, Monday to Friday between the hours of 8am - 8pm.
The role is based at St Marks Hospital and free staff parking is available
Main duties of the job
- To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service Virtual Frailty Ward in collaboration with the UCR VFW Geriatrician and Specialty Doctor

As an experienced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.
