Job summary

We are looking to recruit a parttime 25 hours per week, Band 7 Urgent Care Patient Flow Co-ordinator to work within our Out of Hours Urgent Care service.

Based at our Primary Care Centre at the Royal Berkshire, you will be part of a forward-thinking team of GPs, Practitioners, Nurses and Healthcare assistants, providing high quality out of hours care to the local population.

Our Primary Care Centre is open 7 days a week from 6.30pm until midnight weekdays and from 8am until midnight at weekends.

Our current vacancy is for a nurse/paramedic who can cover Saturdays and Sundays from 10am until 10.30pm and Bank Holidays 10am until 10.30pm.

The postholder will proactively manage and optimise the flow of patients attending the department, as well as managing the flow of patients from outside organisations such as ED and NHS 111, ensuring the patient is seen and managed in the most appropriate place.

Acting as a clinical role model, the postholder will assist in undertaking the initial assessment and triage of the patient, to ensure that the sickest patients within the department are seen and treated in a timely manner.

The postholder will have line management responsibility for the band 5 nurses and band 2/3 healthcare assistants. This will include regular management supervision, sickness/leave monitoring, performance management, and advance adjustments to the rota where necessary.

Main duties of the job

  • To proactively manage and optimise the flow of patients attending the Westcall Primary Care Centre, including those patients referred from Westcall triage, ED, and NHS 111 to ensure that they are seen and managed in a timely manner.

  • Assessing patients on arrival to the department and prioritising them according to clinical need.

  • Support healthcare assistants and nurses with observations of patients' blood pressure, pulse, temperature, and pulse oximetry and escalate patients who have an elevated NEWS score.

  • Undertake investigations as required including ECG, venepuncture, urinalysis and point of care testing.

  • Effectively liaising with a range of inter-professional teams including acute colleagues and primary care colleagues.

  • To take responsibility for maintaining oversight of patients who have had long wait times and to ensure that they are regularly updated and safely monitored in the waiting room.

  • Line management responsibility for the band 5 nurses and band 2/3 healthcare assistants. This will include regular management supervision, sickness/leave monitoring, performance management, and advance adjustments to the rota where necessary.
