The post holder will be a key member of the Care Home Support Team (CHST). CHST team includes a Pharmacist, Registered General Nurses, Registered Mental Health Nurses, Palliative care specialist nurse and Therapists.
The post is for a senior registered general nurse which includes expertise in palliative care/end-of-life care, who will be responsible for holding a patient caseload and enhancing knowledge, skills and confidence to staff working in care homes for residents approaching end of life. Working closely with the wider multidisciplinary team to promote and ensure effective delivery of palliative and end of life care for care home residents.
Based at Cremyll Road in Reading you will be a team player working across Berkshire West. Working across elderly and learning disability care homes in Wokingham, Reading and West Berkshire locality. The role is flexible with an ability to work from home.
CHST works 5 days a week, Monday to Friday, 9-5. This permanent post is 22.5 hours a week. Working days to be confirmed upon offer, however Tuesday would be a required working day.
Main duties of the job

Provide specialist advice and support to care home residents, carers and all health care professionals using advanced knowledge of palliative and end of life care, including specialist interventions at end of life, pain and symptom management, psycho-social needs, end of life care and ethical dilemmas.
