We are pleased to be able to offer an existing opportunity for a band 7 Senior Community physiotherapist role working in the Reading area. The post holder is a Senior Practitioner with the clinical skills and expertise and knowledge to support community reablement /community Physio/Falls service and 2-day UCR pathways. This is an exciting time to join BHFT, as we develop services to work in an integrated way within the community in reading and across Berkshire West.
The successful candidate will be required to provide high quality, responsive services working as an integral member of the intermediate care team and wider Integrated Community team. You will work closely with social services and voluntary organization for adults with a wide range of physical impairments and mental health conditions.
The successful candidate will work autonomously under the guidance of the team manager (as required) having a clinical responsibility for a designated caseload as well as supporting the clinical leadership of the team and management responsibilities.
Reading Intermediate Care is a team of physiotherapists, occupational therapists, senior therapy assistants, triage coordinators and administrators.
This supportive multidisciplinaryteam workwith patients across Reading in close communication with social services, voluntary agencies, and GPs.This post involves 7 days working within a shift pattern within the intermediate care service.
