Urgent Community Response (UCR)/Virtual Frailty Ward

The role will be a key member of the multidisciplinary team; working alongside the Geriatrician, Nurse Consultant. Advanced Clinical Pharmacist for Care Homes, Advanced Care Practitioners, Nurses and Therapists, providing expert medicines advice, guidance, recommendations, medication optimisation support and any necessary training on medicines-related issues to the team, to prevent unnecessary admissions to hospital.

You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and that effective processes and procedures are maintained across the service.

The post holder will work 37.5hrs per week, 5 days a week, between the hours of 9am - 5pm.

The role is based at Cremyll Road, Reading and staff parking is available locally

Main duties of the job

  • To assist with medication aspects of the UCR and Virtual Frailty caseloads, ensuring all legal and professional aspects, as well as robust governance processes are met.
  • To work closely with the MDT carrying out comprehensive medication reviews for elderly/frail patients, as needed.
  • To work flexibly as services develop to meet the NHS Long Term Plan Ageing Well requirements.

  • To work closely with the relevant community teams, primary care colleagues and GPs to ensure that seamless pharmaceutical care is provided throughout the patient's journey.

  • To support the UCR and Virtual Frailty teams with medication queries.

  • To assist with the development of and implementation of prescribing policies, standard operating procedures, protocols and guidance related to medicines, as required

  • To carry out medicines-related audits or projects relating to UCR/VFW prescribing practices

  • Work flexibly in response to service need across our multiple sites
