Job summary
We are looking to recruit a Band 7 clinical lead Nurse for the Community Specialist Heart Care Nursing team across Berkshire West, to lead a team of Specialist Community Heart Care Nurses and with them effectively manage a collaborative approach to reducing avoidable admissions to hospital and to assist in reducing delayed discharge of patients with Heart Failure.
To deliver evidence-based Heart Failure interventions through a case management approach to patients after an admission for Heart Failure or where Heart Failure is a concomitant condition, or as nominated by GP's or other professionals, to reduce hospital admissions.
Act as a link nurse to a designated community nursing team(s) in service districts to support teams to provide evidence-based interventions to ensure required standards are met. To assist the development of services for the patients with Heart Failure in collaboration with the Royal Berkshire Hospital Foundation Trust and GPs. To ensure the collection of outcomes data and the production of reports related to the delivery of the Community Heart Care Services
Main duties of the job
- To be responsible for the Heart Function component of the Berkshire West Community Based Nurse led service.
- To accept autonomous clinical responsibility for a designated caseload of patients with Heart Failure Berkshire West localities, providing high quality, responsive, specialist heart failure community services in patient's homes or other clinics
- Responsible for prioritising and monitoring referrals to the Heart Failure service for patients diagnosed with Heart Failure
- Lead a team of Band 6 nurses, with regular 1 to 1's, appraisal process and MDT/clinical supervision.
