Come and join our CQC rated 'Outstanding' Trust as a District Nursing Sister/Community Nursing Sister/Charge Nurse for the Bracknell Community Nursing Team. As a District Nursing Sister/Community Nursing Sister/Charge Nurse you will manage a clinical caseload under the delegation of the team leader as well as exercising leadership and management of a defined community DN team to ensure good clinical practices and high standards of care are maintained in order to meet service demand. Working within our Community Nursing Team you will support, assess, and treat patients with a variety of health needs, delivering high quality, evidence-based practice. You will have a passion and enthusiasm to care for patients in their own homes.
Main duties of the job
- Maintain required competencies, as identified in the Band 6 Community Nurse competency framework, in order to effectively undertake patient assessment, care planning and immediate care needs.
- Support staff to complete holistic assessment and reassessment of all health and nursing needs, ensuring that every patient has an overview of health and nursing need recorded.
- You will manage a complex and diverse caseload of patients
- Support teams to provide information to patients, their families, and carers both formal and informal, so that they can make informed choices about current and future care needs.
- Provide clinical support, leadership, and guidance to all staff to ensure that evidence-based care and clinical practice are maintained in Community Nursing Teams.
