Job summary

We are looking for an experienced, dedicated, and compassionate Mental Health nurse to join our friendly Bracknell Memory Clinic for two days a week (15 hours).

Your base would be church Hill House, which has access to free parking facilities. There is also the opportunity for hybrid working with a balance of office based and remote working.

To maintain benchmarked standards following inducted you will be expected to complete initial assessments of patients who have been referred for a memory concern and review their care plans.

Main duties of the job

You will be responsible for undertaking comprehensive assessments in a range of settings including patient's own home, considering the person's physical, psychological, social and spiritual needs. Woking with the Multidisciplinary team develop an agreed care plan with the service user and where appropriate include the view of the families and carers. You will manage a case load and have the expectation to use your knowledge of a variety of therapeutic interventions, adhering to trust policies and maintain accurate clinical documentation as per guidance.

  • RMN or equivalent Professional Qualification (OT, SW)

Desirable

  • Completion of post-graduate courses related to specialism e.g. Advanced Dementia Care, Cognitive Behaviour Therapy and Communicating & Caregiving in Dementia etc
  • RGN (i.e. dual qualified)
  • Mentorship or equivalent.
  • Diploma in Dementia Studies or willing to undertake
