Senior Specialist Nurse, Care Home Support Team

Job summary

The post holder will be a key member of the Care Home Support Team which is the proactive branch of our Urgent Community Response multidisciplinary team. The Care Home Support team includes a Consultant Geriatrician, Pharmacists, Registered General Nurses, Registered Mental Health Nurses and Therapists.

You will be a team player within the West Berkshire locality and expected to work collaboratively with staff from all disciplines whilst maintaining high professional standards. The post holder will be required to support care homes across West Berkshire Newbury, Thatcham, Hungerford and Lambourn. The team work flexibly with visits to care homes, use of Teams, working from home, meetings at trust sites.

The proactive branch works 5 days a week, Monday to Friday, 9-5, the post holder will be required to work 30 hours, hours can be discussed.

Main duties of the job

  • To build good working relationships with the care homes in the area.
  • Advise care homes on management of clinical issues as required. This will be dependent on the skill set of the successful candidate and service needs.
  • Provide education to care homes via TEAMS or face-to-face on areas of care the home has identified as a need. This could be formal or informal with role modelling/upskilling.
  • Provide a holistic assessment to aid medical diagnosis, management, and treatment of complex residents in the care home environment and prevent unnecessary hospital admissions or placement failure.
  • Work in collaboration with other members of the MDT, Social Services, Care Quality and ICB.
  • Work alongside the RMNs/RGNs in the other localities on innovations to improve practice across Berkshire West.
