Job summary

The Nurse Advisor will provide clinical support, assistance, and professional expertise to both Referral Co-ordinators, professionals, and patients within the Berkshire Hub, with a priority for 2-hour UCR referrals. The service is the single point of access in Berkshire taking referrals from professionals and patients requiring healthcare services for Health, Social and Mental Health services for children.

The Nurse Advisor will offer clinical triage and support where appropriate. This may range from signposting the referral co-ordinator/caller to the appropriate service, making a direct referral, or offering clinical advice.

The post primarily operates as shifts between 08.00-18.00 Monday to Friday. There is some home working expected and equipment will be provided.

This role is working in a busy environment and includes operating several different IT software.

Main duties of the job

  • Provide high quality triage of referrals to ensure seamless patient pathway from referral to treatment.
  • Provide support and assistance to Referral Co-ordinators in managing difficult telephone contacts.
  • Provide advice to health/social care professionals and patients.
  • Manage workflow through the Hub to ensure key performance targets are met
  • Knowledge of community health services to facilitate admission avoidance and ensure referrals are directed to the appropriate service.

The "must haves" for you to be considered for this role:

Registered General Nurse with a minimum of 2 years' experience with a preference to community nursing. Newly qualified would be considered but as a band 5.

Willingness to undertake further training and development.

Evidence of on-going professional development

Excellent written and verbal communication skills
