Are you looking for an exciting new role in a high performing and supportive team? Perhaps you are ready for your next challenge? We have a fantastic opportunity for an occupational therapist to join the West Berkshire Urgent Community Response /Virtual Ward Team (UCR/VW).

The post holder will be a key member of the UCR Team in West Berkshire. UCR is a fast paced 7 day a week community-based service delivered by a multidisciplinary team to adults (over the age of 18 years), who are experiencing a crisis which may result in an unplanned hospital admission without a 2-hour crisis response. Patients can be acutely unwell or in the end-of-life phase.

The role will involve working with members of the UCR team which include Advanced Nurse Practitioners, Support Workers, Multitherapy Assistants, Paramedics, Consultant Geriatrician, Physiotherapist in delivery of the UCR service therapy needs.

As an experienced advanced practitioner, the post holder will be required to work autonomously and manage complex cases at a fast pace to accommodate the urgent needs of patients. This is a multifaceted role assisting patients to remain at home, supporting access to reablement or establishing long term needs and referring onto appropriate services as required.

Training opportunities include completion of the Advanced History Taking and Physical Assessment Course.

Main duties of the job

To undertake the comprehensive holistic assessment of patients with highly complex physical, mental health and social needs, using investigative and analytical skills to formulate individualised management and treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options to formulate specialised programmes of care.

To be able to use research, clinical reasoning and problem-solving skills, including activity analysis in a wide range of highly complex conditions, to determine appropriate actions and formulate specific and appropriate management plans including the setting of time scales in relation to available resources and professional demands.

To carry out treatment activities with patients that have unpredictable physical limitations and difficulties with understanding, due to physical, psychological, cognitive or social problems.

To assess, recommend (where appropriate) provide, fit and evaluate the effectiveness of a range of specialised equipment and aids (including assistive technology) to facilitate patients' rehabilitation in their own home, following either a crisis episode in the community
