Bracknell Community Mental Health Team for Older Adults is officially "Outstanding" (CQC Dec 2015)! Bracknell OPMH services comprise Memory Clinic, CMHT OA, specialist Home Treatment Team, Dementia Advisory Service and Local Authority dementia team.
We are looking for experienced practitioner with highly developed clinical skills and expert knowledge in the treatment of older people with mental health and cognitive problems. You will be responsible for day-to-day coordination of the service provided to older people with mental health needs by community mental health and home treatment team. Clinical lead will provide leadership to the CMHT and HTT and will represent the service with outside agencies in meetings and other events. The clinical lead will act as a role model as well as provide supervision, appraisals and development discussions, education and highly specialist advice to colleagues and other agencies. This is a full time post and your working hours are in general office hours Mon-Fri 9-5pm; however, at times there may be required some flexibility around that. The home treatment team is 7 days service. We apply hybrid working; days working remotely and in office.
Bracknell is mix of urban and rural communities and it is essential that workers have a car and associated insurance to travel around the area to visit patients when necessary. However, in general the geographical area is not large.
Main duties of the job
- To be responsible for a small caseload of patients with highly complex needs, undertaking Care Co-ordination where psychiatric expertise is required to ensure successful engagement with community care services.
- To provide leadership and direction for CMHT / HTT staff and wider MDT as well as when representing the service in other external meetings/deputising the service manager.
- To plan, facilitate, deliver and evaluate specialised person-centred care and treatment for older people with mental health and cognitive problems.
- To provide education, counselling and emotional support to patients and their carers.
- To offer specialist advice and guidance to colleagues regarding appropriate treatment and management of older people with complex mental health needs.
- To liaise effectively with other provider services, ensuring that all relevant personnel are aware of any changes in care/treatment or client need.
- To observe and implement the requirements of the Mental Health Act and other relevant legislation.
- To take a lead role in the improvement and further development of the service provided by the team, including contributing to the production and regular review of policies and procedures relating to the care of older people.
- Utilising an expert knowledge base, to contribute to on-going strategic planning for older peoples' mental health services.
