Are you looking for an exciting new role in a high preforming and dynamic team? Perhaps your ready for your next challenge? We have a fantastic opportunity for a Service Lead for the East Berkshire Urgent Community Response and Virtual Ward Team.

We are looking to recruit an Advance Nurse Practioner (ANP), who will work as part of a multi-disciplinary team to provide advanced clinical assessment skills to patients in their own home and in a care home, who require an urgent response to prevent unnecessary admission to hospital and leadership to the team.

The role will provide day to day management of the East Berkshire Urgent Community Response and Virtual Ward Team multidisciplinary team, including nurses, therapists and support workers, and clinical supervision and training/ education to the nurses and support workers, as required.

You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.

The post holder will work 37.5hrs per week, 7.5 hours a day, Monday to Friday between the hours of 8am - 8pm.

The role is based at The Cottage, St Marks Hospital and staff parking is available

Main duties of the job

  • To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service Virtual Frailty Ward in collaboration with the Community Geriatrician and GP.
  • As an experienced advanced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.
  • The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner
  • As an Independent Prescriber, prescribe and review medications in accordance with NMC guidance, local and national policy guidance.
  • To provide leadership and line management to the Band 8/7/6 nurses and therapists and the Band 3 & 4 healthcare/multi-therapy assistants.

  • Non-medical prescriber
