Job summary

The Community Nursing Team in West Berkshire (Newbury) is an integral part of our Long-Term Conditions and Frailty Services. We are proud to say that our recent CQC inspection rated our services as Outstanding.

The team has a wide range of skill mix from band 3 Health Care Assistants through to band 7 Community Matrons. We have worked hard to establish a service that offers a range of career development opportunities for our staff, which in turn enables us to deliver continuity of care, Job satisfaction and high standards of care.

The post holder will be part of the Community Matron team and will have a leading role in the management of patients with complex long-term conditions, preventing unnecessary admissions to hospital and supporting patients within their home. The post holder will also contribute to the training and development of colleagues, whilst supporting a service two-hour response to patients in crisis.

This development opportunity will enable you to work alongside the Community Matrons to develop your skills and knowledge in managing a complex caseload of patients with long term conditions. You will be supported throughout with training and mentorship to enable you to achieve the Community Matron competencies and develop into a band 7 Community Matron role.

Main duties of the job

  • Supporting the Community Nursing Services to develop and deliver high quality, outcome driven clinical services.
  • Developing strong working relationships with colleagues in general practice, social care and across Berkshire Healthcare Foundation Trust.
  • Developing your level of clinical knowledge, advanced history taking and clinical reasoning skills, including undertaking your Nurse Independent and Supplementary Prescriber Qualification.
  • Having the autonomy and the ability to exercise clinical reasoning and decision making that is critical for the care you need to give.
  • Having the authority to affect change to your patient's life "in real time" through access to all services within the Integrated community services with no barriers.

Education/Qualifications

Essential

  • Registered Nurse (Adult).
  • 1st Level Degree or working towards.
  • Professional qualification (RGN, dual qualification an advantage).
  • Practice Assessor training
  • There is an expectation that the post holder will work towards a non-medical prescriber qualification and an advanced degree/masters, you will be supported within the role to achieve this.

Training

Essential

  • Advanced History Taking qualification or equivalent or a willingness to undertake.
  • Knowledge of long-term conditions

Additional Requirements

Essential

  • Hold a UK Driving Licence.
  • Ability to travel to, and work from multiple sites.
  • Service covers 08:00-19:00 hours, 365 days per year - shift patterns will include some weekends, lates and Weekends.

  • 4 years post registration experience.
  • Experience of supporting people with long term conditions.

Desirable

  • Experience of working as a Band 6 Registered Nurse (Adult).
  • Experience of working in a variety of settings (hospital, primary and community).
