Job summary

The post holder will provide specialist service to advise and promote evidence-based practice in the care of all patients planned to receive nutrition support. Working in partnership with the patient, their families and cares and the wider multi-disciplinary team to provide/facilitate personalized, effective patient care.

The post holder will motivate staff to provide high standards of care by acting as a role model. Provide expert professional and clinical care advice to the patients, cares, and multi-disciplinary team. They will be responsible for providing specialist education and training programmes for health care professionals and other professional groups.

Working independently and alongside the clinical lead for nutrition you will be part of the complex nutritional needs MDT. You will be responsible for supporting the nursing assessments of patients care needs and support with the development, implementation, and evaluation of nutritional programmes of care.

You will provide excellent specialist nursing practice regarding nutritional care specialist support for enteral tube feeding and those receiving Total parenteral Nutrition (TPN).

Act as a positive role model through the provision of effective professional practice and the modelling of the Trust Values.

Main duties of the job

This is a new post you will:

> Set up the Nutrition Nurse Service within WHH hospitals

> Working with the MDT you will support patients who are receiving enteral or parenteral nutrition in hospital.

> You will develop a pathway to support patient's who attend the Emergency Department with feeding issues to help provide care/signposting and prevent unnecessary admissions.

> You will support the Trusts target to ensure all patient's have a MUST assessment on Admission and a minimum of every 7 days.

> You will formulate and maintain a database of all patients referred to the complex nutrition MDT.

> you will coordinate, participate in and ensure actions are documented for all patients referred to the complex Nutrition MDT.

> you will support the ward staff ensuring that monitoring of patients receiving nutritional support is within the expected standards.

> you will develop education and training for patients and staff in regards to nutrition.

> you will be an active member of the Nutrition , Food & Hydration Steering Group.

> You will work as part of the multidisciplinary team to ensure that the Trusts Food & Drink strategy is achieved.
