Job summary
We have a wonderful opportunity for enthusiastic, motivated Registered Nurses to join our fantastic award-winning Acute Care Team. We are privileged to have exciting opportunity to be expanding our current patient safety services and workforce within the Acute Care Team /Outreach service to work at Warrington & Halton Hospital Teaching Hospitals.
The team is highly regarded, supporting both medical and nursing staff in the care of the deteriorating patients and those transitioning from Intensive care to wards.
We are looking for nurses with extensive experience with caring for critically ill patients who wish to seize the opportunity to develop themselves and the service, whilst being given formal and bedside training to support skills.
We are looking for skilled practitioners who are passionate about improving patient safety and the quality of care for patients at risk of deterioration and those post critical illness. Experience in clinical assessment and treatment of the acutely ill patients in acute settings I.E ED, ITU and/ or ward environment are essential attributes for this post.
The successful candidates will be able to work well within a team and be willing to work a mixed shift pattern including days and nights across sites.
Main duties of the job
The post holder is required to have excellent interpersonal skills with ability to liaise effectively with the multidisciplinary team and act as an excellent role model, maintaining a positive workplace culture. They will be expected to work in partnership with the medical lead for the acute care team and critical care, only with other senior nursing and medical teams.
Manage a patient caseload within the agreed protocols, working in partnership with other professionals and referring patients to other specialties or practitioners as required. Respond to medical emergencies.
Assessment of patients to include examination, assessment and diagnosis of patients admitted to clinical inpatient areas. Undertake ordering and follow up investigations for the purpose of diagnosis and act on findings appropriately.
Actively review and assess patients post discharge from Intensive care, making treatment decisions for ongoing care and support.
Maintain adequate and accurate patient documentation in-line with professionals body's requirements for all patients reviewed and where advise is given in any practice setting and contribute to clinical / data collections as required.
