Job summary
An exciting
opportunity has arisen within Warrington and Halton Teaching Hospital
Foundation Trust for a cardiac rehabilitation specialist nurse to join our
Cardiac Rehabilitation Team. Warrington and Halton Cardiac Rehabilitation
Service provides support for patients with ischaemic heart disease and heart
failure using face-to-face and remote systems of working. The service currently
holds Green Accreditation status from the British Association for
Cardiovascular Prevention and Rehabilitation
Main duties of the job
The main duties include reviewing patient for
phases I to III of their cardiac rehabilitation pathway in Warrington Hospital
as well as in Halton Hospital Cardiac Rehabilitation Centre and Health Services
at Wolves (Warrington). This varied role
includes reviewing patients over the telephone, face-to-face in a clinic
setting and supporting exercise groups as well as providing education talks.
The post holder will work as part of a multi-disciplinary team across
Warrington and Halton sites.
About us
The multi-disciplinary Cardiac Rehabilitation Team consists of
cardiac nurses, exercise professionals, physiotherapists, an occupational
therapist and assistant as well as administration support staff. This is a
friendly team which strive to improve service provision within cardiac
rehabilitation for Warrington and Halton residents. As part of Warrington and
Halton Teaching Hospital Foundation Trust, Cardiac Rehabilitation also works
closely with the cardiology team.
