Job summary
- Halton Community Therapy Team (HCTT) is a small, but perfectly formed team of currently 11 individuals; you will make us 12. We are Physios & OTs, supported by our own Admin & Assistants working exclusively on the Community.
- HCTT is based at Halton General Hospital; our patch covers the Borough of Halton (population 129,000) which encompasses the towns of Runcorn & Widnes and includes a few of our local outlying villages that sit within Halton.
- HCTT are an Adult (over 18s) Rehab team which assesses & treats a wide variety of conditions including Neurological, Orthopaedic, Rheumatological, Arthritic, Palliative patients, Fallers & Frailty, Amputees, and generic mobility dysfunctions.
- Referrals are taken from Primary Care EG: GPs, Therapists, Macmillan Nurses, Community Matrons and other specific Nursing teams - and also from Consultant led teams or Therapists from our partners in Secondary care at surrounding Hospitals.
- Our raison d'etre is providing high quality Rehab to patients within their own home; which may include Care Home, thus resulting in therapy gains and improvements to quality of life; and where the knock on effect is reducing the risk of admission to hospital as well as facilitating discharge where appropriate.
Main duties of the job
- To assess & treat patients within their own home, referred with a wide variety of conditions.
- Along with our existing part time B6 Physio to take the lead role in the supervision of students within the service.
- To contribute to the supervision of B5 or Assistant grades of staff.
- To contribute to Clinical Audit and to other forms of Service Development, as appropriate.
