You'll be part of a service that truly puts people first, working in their own homes or in discharge-to-assess units, following a "home first" approach.
You'll have opportunity to rotate across key areas of our service, gaining diverse experience and making a meaningful impact:
- Single Point of Access
- Community Rapid Response / Frailty Team
- Reablement Team
- Intermediate Care Unit
Join our vibrant multidisciplinary team including GPs, Advanced Nurse Practitioners, Physiotherapists, Occupational Therapists, Social Workers, Therapy & Nursing Assistants, Dieticians, Care Support Workers, and Administrators. Collaboration is at the heart of everything we do.

- Carry out holistic therapy assessments and provide clear diagnoses
- Design and deliver personalised treatment programmes that empower patients
- Collaborate with a multidisciplinary team and liaise with external agencies to ensure seamless care
- Contribute to service improvement projects, audits, and in-service training
- Support and supervise students and junior staff, helping shape the next generation of therapists
- You'll need to be adaptable and ready to travel across Halton to meet service needs.

Our service runs7 days a week, from8am to 8pm, including weekends and bank holidays. Youll work on a rota basis, giving you flexibility and variety in your schedule. Part time hours will be considered.
We offer a structured induction program. Career progression support provided through individual job plan, core skills training, mentoring , regular supervision, CPD sessions to become skilled specialist clinicians with a good range of knowledge and hands on experience.
We welcome applicants looking for band 5 to band 6 developmental role.
Our service supports teams health and wellbeing through activities and accommodate flexible working.
