Job summary

An exciting opportunity has arisen for a Band 4 Cancer MDT Coordinator at Warrington and Halton Teaching Hospitals NHS Foundation Trust. The post will be working with the Colorectal Team

Working with our existing MDT Co-ordinators we are looking to add to the team to provide full support in all aspects of this role and to provide MDT Co-ordinator cover across all tumour groups.

Within this you will be responsible for data collection and input, facilitating MDT meetings and tracking patients referred to the Trust with a suspected and confirmed cancer diagnosis.

You will work closely with the Clinical Nurse Specialists and the Multi-Disciplinary Teams within the specialties as well as the rest of the Cancer Services Team.

The Cancer Management team is committed to the personal development of team members and there will be opportunities for development within this role.

Enthusiasm, commitment and flexibility are required for this role along with good customer service and excellent organisational and communication skills. Ability to work under pressure in a fast paced environment with excellent attention to detail is absolutely essential.

Main duties of the job

The post holder is responsible for the facilitation of Multi-disciplinary team meetings ensuring MDT meetings run effectively and are well structured.

To ensure that patients are discussed according to evidence-based protocols and in line with the MDT operational policies.

To provide administrative support to the multi-disciplinary team meetings preparing and maintaining all related documentation and information.

To ensure patient treatment plans and multi-disciplinary team decisions are recorded onto designated proformas, transferred into medical notes and are updated on the relevant electronic systems.

To input outcome data into local systems, in a timely manner, during the MDT or immediately after the MDT, ensuring data accuracy at all times.

To be involved in the recording of data that supports the collection of the Cancer Outcomes Data Set, Cancer Waiting Times, audit data sets for cancer services.

Responsible for tracking patients through their pathways to support the delivery of performance against all cancer access targets.

Provide support documentation of any breaches through investigating the causes.
