Job summary
The opportunity has arisen to join the Enhanced Reconnect service. This is a FTC of 12 months (32 hours per week) although secondments will also be considered.
The service aims to work with high complexity and high-risk offenders who have complex health needs that may impact their emotional wellbeing and / or risk of re-offending.
The post will contribute to reducing health inequalities and address health-related drivers of serious and high-risk offending behaviours. The post will offer trauma-informed, psychologically-led interventions, structured support, advocacy, and signposting. The main aim is to help those leaving prison to engage with community- based, health and social support services, to reduce social exclusion and thereby reduce their health-related offending behaviours. The role will also be expected to work with and support community-based services to work with high complexity and high-risk individuals, where necessary.
The service covers a large geographical patch covering from Cornwall up to and including Gloucestershire. As such travel will be a daily part of your work as your caseload could covers different parts of the patch. Practitioners have designated areas to cover but there is still travelling involved.
Main duties of the job
As part of the Enhanced Reconnect service you will work alongside a small team of health care professionals and probation staff. You will hold a caseload of people who initially will still be in prison and you will support them in their transition from prison and up to 12 months post release. This will involve working with the person directly offering psychological interventions, practical support and guidance and indirectly with other agencies to help them access mainstream services and support.
You will be supported by the clinical lead who will provide monthly clinical supervision, reflective practice and regular case formulation meetings.
Training will be given in delivering psychological interventions both as a team and individually dependant on service need and individuals preferences.
