Job summary

We are seeking an experienced Senior Clinical Nurse Specialist to join our West of England Specialist Gender Identity Clinic Team at Devon Partnership NHS Trust.

In this pivotal role, you will provide specialist assessment and interventions for transgender and non-binary patients, acting as a named professional responsible for both initial assessments and ongoing follow-up care within an identified caseload.

You will monitor gender affirming hormones for trans male, trans female, and non-binary individuals, and work collaboratively with the Medical Lead and Service Manager to continually develop the service. As a Transgender Healthcare Professional, your practice will align with NHS Englands national service specification, supporting patients and, where appropriate, their families through direct and indirect clinical care.

Applicants must be an experienced registered nurse (Mental Health/Learning Disabilities or General) with current NMC registration and NMP qualification. You will demonstrate strong clinical and managerial leadership, providing education and supervision to junior staff and colleagues, and operate autonomously within professional guidelines and service policies. Specialist skills in assessment, needs analysis, and treatment planning within a multi-disciplinary framework are essential.

Main duties of the job

  • The post holder will communicate in a skilled and sensitive manner information concerning the assessment, formulation, and treatment plans of patients under their care.
  • Ability to assess trans individuals with complex and specialist mental and physical health needs.
  • To independently prescribe following verification of eligibility by a Lead Clinician
  • Demonstrate an ability to carry out a comprehensive assessment of health, wellbeing, and social needs.
  • The post holder will provide advice concerning the assessment, care, and treatment of patients with gender incongruence. Develop specialised programmes of care.
  • Undertaking the role of the Senior Clinical Nurse Specialist and hold a caseload to develop individualised care pathways, with patient involvement, whilst providing advice and guidance in relation to transgender healthcare.
  • Liaising with and referring to other professionals, as appropriate within and outside of the clinic to ensure effective care and treatment for patients attending the clinic.
  • Provide highly specialist advice to patients/clients, carers, relatives and other professionals
