Communication and Working Relationships
Be linked to and work closely with the community clusters to facilitate a seamless service between Devon Partnership NHS Trust and partner agencies.
Work closely with key external personnel including, Police, Housing, Voluntary and non-statutory agencies and a wider Health and Social Care Community.
Ensure close liaison with primary health care staff and commissioning officers regarding referral criteria into the services and quality standards and ensure targets from the National Framework are met.
Liaise with professional stakeholder groups and interface service providers and agencies to develop mutually helpful working relationships.
Undertake formal counselling either on a 1:1 or a group basis.
Present information, some of which may be contentious, to staff groups, service user groups, carers groups and other stakeholders using a range of techniques and media.
Investigate complaints, grievances and performance issues taking remedial action as necessary.
To be a part of the relevant professional leadership group (nursing, OT, social work) and attend its meetings.To link with the relevant professional lead (nurse, OT, social worker) within Devon Partnership Trust.
Planning and Organisation
Actively participate in agency forums to ensure the identification of need and the development and delivery of integrated local services in line with local need.
Ensure that activity is delivered in accordance with agreed quality standards and to service level agreements.
Pro-actively lead on business continuity within their team. Working with other managers to ensure that there is a robust business continuity plan for their service that links in with the Trusts Major Incident Plan.
Ensure adequate staff cover to provide safe and effective practice.
Responsibility for Patient/Client Care, Treatment and Therapy
Promote and develop high standards of clinical care.
Develop and implement care plans, ensuring active involvement of service users their carers and advocates and to promote, where ever possible, personalised solutions with service users and their carers .
Ensure effective care pathways are in place for transition between Older Peoples mental health and other services eg Networks and Learning Disability Services.
Ensure the availability of choice that meets the needs of the local population.
Work closely with other teams with the Trust and the Voluntary, Statutory and independent sectors to ensure effective discharge planning which incorporates implementation of key success criteria for NICE guidance eg CPA, Preventing Suicide, and Information Sharing etc.
Implement a formal and active system of caseload review, caseload management and pro-active care management that will enable achievement of local and national targets.
To work with the relevant Carers legislation and ensure that Carers are both involved and supported.
Policy, Service, Research, Innovation and Improvement
Take a lead on specific areas of service effectiveness across the network to ensure the development of local expertise, ongoing service improvement and professional monitoring of standards.
Undertake programmes of audit and research within the team and implement the findings.
Assist teams and individuals to negotiate changes in practice required to meet ongoing service developments.
Identify and implement best practice in the delivery of Mental Health services within the team and disseminate these throughout the service.
Participate in the maintenance of current services and future developments within the locality eg Care Pathways development.
Initiate service developments and projects.Plan provision of local services in conjunction with the service and other managers.
Participate in the service governance and research agenda within the locality e.g. Clinical Audits, NICE and Standards and Evaluation Research.
Responsibility for Finance, Equipment and Other Resources
Responsible for the management of health budgets within the team, both pay and non-pay, to ensure the optimum use of available resources.
Advise team members on matters relating to care management, Section 117 and Continuing Healthcare (CHC). Participate in relevant funding decisions as agreed with line manager.
Liaise with the appropriate departments on financial matters relating to care for individual service users funded in whole or part by the NHS.
Responsibility for Supervision, Leadership and Management
Lead and operationally manage a team of staff ensuring that they operate within policies and procedures of the relevant Trust.
Manage the overall day to day work load of the team dealing with staff and performance issues as necessary.
Recruit and select staff within the team according to resource requirements. Ensure that new staff receive an induction programme.
Ensure implementation of appropriate management and clinical practice supervision arrangements for all staff and provide effective management arrangements for all staff.
Ensure that all staff receive training in accordance with the training plan, minimal requirement for their professional registration and competencies for the Care Programme Approach and recovery competencies. Ensure completion of all PDRs to support staff development.
Ensure that all staff are compliant with the Mental Health Act, Mental Capacity Act, Care Act, Safeguarding Adults and other relevant legislation.
Contribute to the development of the effective workforce planning and development for their function within the network.
Information Resources and Administrative Duties
Ensure that activity data is collected and presented to the relevant management forums in their agreed format.
Any Other Specific Tasks Required
Monitor standards and ensure Devon Partnership Trust policy compliance including activity data, manual and electronic record keeping and effective care planning.
Ensure robust risk management systems are in place:
Have an ongoing awareness of high clinical risk situations within the relevant teams and advise clinicians on actions to be taken to respond to risk.
Review all incident reports and complete serious incident reports.
Chair meetings relating to clinical management of individual clients where high levels of risk are involved.
Responsibility for Safeguarding Vulnerable Adults (SGA):
To ensure that safeguarding adults is at the centre of own practice and undertake training to advanced level.
To ensure a high level of understanding about all relevant safeguarding policies, practices and procedures throughout the team.
To undertake investigations, case-manage and joint-work complex SGA cases.
To liaise with the SGA team and to chair SGA strategy meetings and to support and monitor staff in this area of work.
