Job summary

We are looking for a Community Mental Health Practitioner who are Registered General Nurses (RGN) or Registered Mental Health Nurses (RMN) to play a pivotal role in the delivery of mental health care within Exmouth Core Adult Team.

A Community Mental Health Practitioner supports people with mental health conditions outside of hospital settings - in their homes, clinics, GP surgeries, supported housing, or community centres. The goal is to empower clients to live as independently and safely as possible while managing their mental health.

The focus will be providing continuous assessment and treatment to people with mental health difficulties and their carers. This will include recovery keywork intervention and care management functions, liaison with statutory and non-statutory organisations.The post holder will deliver high quality mental health services andwill be an effective member of the multi-disciplinary team.The role involves managing a defined caseload.

Main duties of the job

The role supports recovery-focused, person-centred services that promote independence, social inclusion, and continuity of care. You will work as part of multidisciplinary community mental health teams (CMHTs), provide assessment, treatment, risk management, and ongoing support to individuals experiencing a range of mental health difficulties, including severe and enduring mental illness.

The CMHP role requires a combination of clinical expertise, therapeutic skills, professional judgement, and collaborative working. Practitioners must balance mental health and physical health needs while ensuring care is delivered safely, ethically, and in line with professional and legal frameworks such as the NMC Code, the Mental Health Act, and safeguarding legislation.

  • An appropriate up to date professional mental health qualification eg RMN, OT or social worker on the appropriate
  • professional body register and hold current registration with no restrictions.

Desirable

  • Training to level 2 in Safeguarding children and young people: roles and competencies for Health Care staff.
  • Post registration training in relevant field.
  • Mentorship training.

Experience

Essential

  • Postgraduate experience in at least two inpatient settings or substantial experience working within a mental health and/or learning disabilities setting.
  • Managing a caseload of clients with a range of mental health problems.

Desirable

  • Demonstrates sound theoretical knowledge of mental health practice;
  • Working within a community setting in either the statutory or voluntary sector.
