Job summary
There are two Senior Dementia Practitioner roles available, 1x
0.6wte and 1x 0.8wte. Both roles are based within the post diagnostic team and will work from Brookland Hall. These city-wide roles will
focus on delivering high quality, individualised support to service users and
carers, encompassing a wide range of 1:1 and group interventions. The post holders
will work collaboratively with the Psychology team, to deliver bespoke
interventions for complex service users andcarers.
The roles will work alongside a Clinical Support
Manager to support development and implementation of the services Preventing
Well agenda, engaging with stakeholders and others in defining and
delivering this pathway. This is an exciting development for the service as we engage in initiatives to promote good brain health, maintaining
function and independence and reducing risks for developing dementia.
The post holders will be expected to have experience of
setting up and running groups, goal setting skills and knowledge and implementation
of outcome measures. Experience of using cognitive rehabilitation
is a desirable skill to promote independence and adaptation to life changes.
Experience of clinical supervision of support staff would be
of benefit to the role, recognising the value of leadership skills in supporting own and team development.
The post holders will contribute to the services access
point rota on a regular basis, answering and responding to clinical calls.

In summary, the post holder will develop, deliver and evaluate the interventions conducted by the post diagnostic team and those under the Preventing Well Agenda. You will be expected to carry a complex clinical caseload and support the Dementia Support Workers, in a clinical capacity.
