Job summary

Devon Partnership NHS Trust is offering an exciting Senior Community Mental Health Practitioner Development Opportunity (Band 5 to Band 6) within the Honiton Adult Core CMHT.

This role is open to Mental Health Nurses, Learning Disability Nurses, Occupational Therapists, and Social Workers who are motivated to develop their skills and progress their career within community mental health services.

As part of the team, you will be supported to develop and deliver core psychological interventions within the treatment pathway. This post offers a structured development pathway, enabling suitable candidates to progress by completing a set of defined competencies. Through this process, you will enhance your skills and experience, working towards meeting the requirements of a Senior Mental Health Practitioner role and potential progression to a Band 6 position.

Main duties of the job

The role would be delivery of 1-1 working with a mixed complex caseload. Working through formulations and care planning using the new personalised care plan approach and assessment tools. You would also be encouraged to complete assessments at first point of contact with the service as part of your development within the team. You can be supported in learning to deliver group interventions and the development of new ideas.

Key Responsibilities:

  • Provide high-quality, person-centred mental health care to individuals in the community.
  • Lead and supervise a team of community-based practitioners, offering guidance, support, and mentoring.
  • Assessment and formulation work
  • Delivering group interventions
  • Attending ward rounds, tribunals, safe guarding
  • Collaborate with multidisciplinary teams and external agencies to ensure holistic care is provided.
  • Support the development of care plans and assist in the delivery of therapeutic interventions.
  • Take a proactive role in the development and delivery of training for junior staff and students.
  • Ensure the highest standards of care and compliance with relevant policies and procedures.
  • Contribute to the continuous improvement of mental health services and service user experience.
