Medicine Optimisation Pharmacy Technician

Job summary

Come work for an outstanding specialist mental health medicine optimisation team, who are focused to empower patients in their medicines decision. We offer a clinical facing patient role across our specialist inpatient wards, community teams and clozapine clinic throughout the Devon area. This post holder will focus on delivering this service through our clozapine outpatient clinic, Exeter based wards and Exeter, East and Mid community setting as required. We offer and support outstanding opportunities to further develop your clinical skills as a pharmacy technician and share best practise outside the organisation. Main duties of the job

Main duties as a DPT medicine optimisation pharmacy technician will include:

  • Provide cover to the clozapine clinic
  • Provide cover as required to inpatient wards
  • Provide cover to community mental health teams/staff as required
  • Medicines reconciliation in line with NICE guidance and local SOPs
  • Manage the ordering, monitoring, storage and destruction of stock and named patient medication
  • Carry out EPMA duties to reduce risk of errors by the prescriber.
  • Provide regular face to face patient counselling
  • Assess appropriateness of complex specialist treatment and liaise with nursing staff, pharmacy and medical staff
  • Liaise with pharmacies to ensure timely and accurate supply of medication
  • Regularly check electronic medication charts to inform performance indicators and ensure charts meet service standard
  • Quarterly CD audits, FP10 & storage audits with the ward
  • Quarterly CD, FP10 audits & storage audits with Community Mental Health Teams
  • Team player within the medicines optimisation team

  • NVQ level 3 and BTEC in pharmaceutical science or equivalent
  • Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC)
  • SWMIT/PWDS Accredited Medicine Optimisation Training programme levels: Foundation & Intermediate or equivalent.

Desirable

  • Accredited checking technician
