As part of a multi-disciplinary team, you will play a key role in delivering high-quality care through holistic assessments and reviews for individuals with potentially complex mental health needs. You will support the team in triaging referrals from a variety of sources and work closely with wider multi-agency partners to ensure seamless transitions of care.
This role also involves delivering brief interventions, which may include psychosocial support, signposting, or onward referrals to appropriate services. You will be instrumental in helping individuals access the right support at the right time, contributing to improved outcomes and recovery.

Communication and Working Relationship Skills
- Work closely and co-operatively with others to ensure safe and timely provision of evidence based interventions.

Responsibility for Patient/Client Care, Treatment and Therapy
- Working within a locality service and through participation in the local referral and care pathway, provide clinical assessment and appropriate triage with a view to risk assessment, risk management, safeguarding, sign posting, treatment or onward referral in response to identified patient need.
- Provide front line assessment to clarify the service most beneficial to the patient. This assessment may be undertaken jointly with other statutory or non-statutory members of staff.
