Devon Partnership NHS Trust is seeking a committed and compassionate Community Support Worker to join our dedicated Baywide team based at the Chadwell Centre in Torbay.
Under the daily supervision of qualified mental health practitioners, you will play a key role in a multi-disciplinary team, working collaboratively to deliver person-centred, recovery-focused care for individuals experiencing a range of mental health difficulties in the community.
As a Community Support Worker, you will:
Work closely with clients in their own homes and community settings to support recovery and independence.
Help prevent hospital admission and assist in safe, timely discharges back into the community.
Engage across agencies, including Primary Care, Complex Care Teams, and Third Sector organisations, to coordinate effective care.
Implement and monitor individual recovery care plans under the guidance of mental health practitioners.
Undertake a significant monitoring role, providing valuable feedback and updates to the wider clinical team.

Main duties of the job
- To communicate effectively with people using services taking into account any communication difficulties they may have and responding appropriately.
- To participate in, support and maintain positive interventions and planned activity programmes to improve the quality of life and mental state of the person using services.
- The post holder will be aware of and work within their own limits, and competencies.
- Take part in the constant assessment of risk and management of risk in conjunction with the qualified staff.
- To liaise appropriately with other Agencies as part of supporting the delivery of care.
- To independently or jointly monitor a persons mental health to ensure safety. This may require intensive input and /or regular monitoring as directed by the mental health practitioners within the team and in accordance with the recovery care plan.
