Job summary

We are seeking to recruit an enthusiastic, highly motivated Band 5 Mental Health Practitioner to work within the East & Mid Devon Home Treatment Team Based at Wonford House.

We are offering an opportunity for a Band 5 clinician to develop their knowledge and skills with a view to achieving a Band 6 role after a minimum of 12 months practice.

The primary roles and purpose of the team is to provide home treatment, avoiding hospital admission where possible and if admission is indicated, we work with clients throughout this time and endeavour to facilitate early discharge.

The post holder will deliver high quality mental health services. The focus will be providing assessment, risk planning and a treatment plan to people with mental health difficulties and their carers. The post holder will be an effective member of the multi-disciplinary team.

The successful candidate will need to be able to have the ability to travel around the locality to meet the requirements of this role.

We welcome applications from newly qualified applicants and also applications from Trainees due to qualify in the near future.

Main duties of the job

You will become part of a multi-disciplinary team creating care plans and strategies that deliver recovery-focused care for people with complex mental health needs. You will play a key role in a dynamic multidisciplinary team, working with multiple agencies on a daily basis. You will be confident working without supervision, with the knowledge to devise individual recovery plans for the people you see. Its important that you can communicate effectively, not only with your colleagues, but the people you care for, their families and carers. You will be at the centre of their care.

You will be passionate about making a difference to peoples lives. You will be responsible for managing and directing the care of some of the most vulnerable people we are responsible for, many of whom will have complex and ongoing needs. You will use your skills and experience to develop packages of treatment that focus on recovery. We call it inspiring hope together.

You will be an experienced mental health professional with a professional mental health qualification (RMN, RGN, RLDN, OT, Social Worker). You will be currently registered with a professional body.
