Dementia Support Worker

We are excited to announce a new opportunity within our well-established and supportive Older Peoples Mental Health team based in Bideford, North Devon. Were looking for compassionate and committed Dementia Support Workers to join us in making a real difference in the lives of people living with dementia, and those who care for them.

About the Role

As a Dementia Support Worker, you will play a vital part in supporting people following a diagnosis of dementia working with them and their carers throughout their journey.

Main duties of the job

You will:

  • Hold a caseload of clients, providing direct support to help them live well with dementia.

  • Carry out visits in people's own homes and care home settings across North Devon.

  • Offer tailored support to those with more complex presentations, including working closely with carers and families.

  • Use the DiADeM (Diagnosis of Advanced Dementia) model to carry out assessments, with guidance from our experienced multidisciplinary team.

  • Collaborate closely with professionals across the team and link in with local dementia services and voluntary sector partners.
