The North Devon Older Peoples Mental Health (OPMH) Community Team is seeking a Specialist Clinical Practitioner to join our dynamic and supportive service.

About the Role:As a Specialist Clinical Practitioner, you will be working autonomously and as part of a skilled multi-disciplinary team to provide comprehensive assessments, formulate diagnoses, and deliver tailored care for older adults presenting with a range of functional and organic mental health conditions.
This role will include diagnostic responsibilities and participation in clinical decision-making around psychotropic medication, including initiation, titration, and review in collaboration with our medical colleagues and prescribing professionals.
Main duties of the job
- Conduct full biopsychosocial and mental state assessments
- Formulate and communicate psychiatric diagnoses
- Make evidence-based decisions around medication, including initiating and adjusting treatment
- Provide leadership in complex case management and risk assessment
- Deliver and oversee person-centred interventions under CPA or equivalent
- Support families and carers with guidance, education, and signposting
- Work closely with GPs, Primary Care, Memory Clinics, and social care teams
- Contribute to service improvement and staff development within the team
