An exciting
opportunity to join the LDAP keyworker pilot.
The Learning Disability and Autism team has been created as part of a
key worker pilot designed in partnership with NHS England. This pilot delivers
a new way of supporting CYP with Autistic Spectrum Disorders (ASD) and/or a
Learning Disability (LD) and their family/carers. The key worker roles will
enable CYP and their family/carers to get the right support at the right time
and ensure local systems are responsive to meeting their needs in a holistic
and joined up way.
The pilot is
part of the NHS Long Term Plan which aspires for all CYP with ASD, LD or both,
in an inpatient setting, to have a named keyworker by 2024. Devon is one of 13
national key working pilot sites implementing this strategic role.
We
are looking for a Band 4 Assistant Practitioner who has the skills and experience of working with
this cohort of young people, or a keen interest in developing skills and has
transferrable skills, and who is interested in joining this innovative and
successful team contributing to its ongoing development.

Main duties of the job
The teams are made up of
Senior Practitioners (Band 7) and Assistant Practitioner
(Band 4) with business administrative support. The teams work together to share
expertise and skills, attend combined training events and share staff supervision.
The teams are trained in
Positive Behavioural Support and will be offered Adaptive Mentalisation Based
Integrative Therapy. They will link into the capabilities frameworks for
working with individuals with ASD and / or LD to ensure they have an adequate
skill base and knowledge in these fields. These models will guide them in
adopting a team around the key worker model of care, where CYP and their
family/carers have a trusted and accessible worker who gains specialist advice
and support from all the other professionals involved in their care.
The post holders will work
across education, health and social care to enable whole system change to embed
principles of transforming care and SEND reforms to improve the experience of
CYP and families.

We are
looking for a Band 4 Keyworker Assistant
Practitioner who can:
Work flexibly within 37.5 hours per
week, which might include some weekends, bank holidays and evenings as the
pilot develops.

Contribute to relevant placement planning
discussions with a key role in alerting systems if instability of placement occurs.
Be
part of facilitating a lead, in ensuring CYP and their family/carers receive
specialist advice and support from one trusted adult who will navigate and
coordinate advice and treatment for other professionals involved with their
care.
Provide family focused support and challenge
to system partners when necessary.
Work
within a multi-organisational landscape, working closely with systems to
develop services able to deliver care packages to suit the individual needs of
the CYP and their family/carers.
Ensure all CYP have a clear personalised care
plan to promote their development, well-being and protect them from harm.
Leading on coordination of support at pace to avoid admission to hospital
Where hospital admission is necessary the
postholder will ensure ongoing involvement throughout the inpatient period,
facilitating timely discharge, well managed transition and full implementation
of CETR recommendations for community support.
Offer emotional and behavioural support and
informed advice to CYP and their family/carers based on shared knowledge and
experience of working with CYP and family/carers with complex needs.
Attend and contribute to CETR and LEAP and
tracker meetings ensuring recommendations/actions are completed in a timely manner.
Be part of a team which creates and encourages
a culture which prioritises keeping CYP within the home and local community by
persuasively communicating with a range of teams and services.
Ensure seamless, well-coordinated transitions
between multiple systems.
Advocate on behalf of CYP and their
family/carers where required to ensure the necessary support is in place to
enable them to remain within or discharged into the community and promote
joined up, consistent care.
Raise awareness/lead by example when educating
the wider system on how to change the conversation so the focus is much more
positive for CYP and their family/carers when speaking to other professionals.
Work across pathways and geographical areas as
required, according to need and demand.
Be
part of a developing a team approach in the development of high-quality
services through the use of audit evidence-based interventions, goal-based,
collaborative practice and Routine Outcome Measures.
Ensure
ongoing participation of service users remains central and core to service

- Foundation Degree in Health and Social Care or qualification to diploma level or equivalent
