Job summary

This is an exciting opportunity for a compassionate Mental Health Practitioner to join the North Devon Rough Sleepers Team on a fixed term contract up to 31st March 2025.

Providing recovery-focused mental health care and support to those who are homeless or at risk of homelessness, liaising with both statutory and voluntary agencies to ensure best outcomes for the service user.

You will be working as part of the core community mental health team, providing in-reach to the multi-disciplinary Rough Sleepers Team based in Barnstaple. You will work alongside health professionals, drug and alcohol workers, housing professionals and voluntary services, engaging with a vulnerable individuals who present with complex health and social care needs such as drug and alcohol dependency and mental health problems.

We are looking for a non-judgmental, skilled and dynamic individual with a commitment to providing high standards of care for this vulnerable group. You will need to have excellent communication skills, the ability to work in collaboration with individuals and other professionals, an excellent problem solver and have the ability to work autonomously.

Experience of working across voluntary services specifically with those experiencing homelessness is desirable but not essential.

Main duties of the job

Main duties of the job include

  • As part of a wider multi-disciplinary team, provide recovery-focused mental health assessment and interventions to build resilience as well as external services; as appropriate.
  • Act as the patient's advocate, working in a proactive, flexible and opportunistic approach in order to promote independence and self-care; as appropriate.
  • Undertake a comprehensive range of activities and have key delegated responsibilities for defined areas of care as part of the wider team, referring as necessary to other services in accordance with established protocols.
  • Where appropriate be responsible for gaining valid informed consent for relevant assessment and associated intervention processes.

  • - Postgraduate experience in at least two inpatient settings or substantial experience working within a mental health and/or
  • learning disabilities setting

Desirable

  • - Managing a caseload of clients with a range of mental health problems.
  • - Demonstrates sound theoretical knowledge of mental health practice;
  • - Working within a community setting in either the statutory or voluntary sector
  • - Providing clinical supervision.
  • - Experience of working in a multi-disciplinary and multi-agency environment.
  • - Demonstrates experience of, and relevant professional practice qualification in mentoring/assessing students and learners
